Getting Started

If you’re interested in starting down the path to wellness, I offer a free 15 minute consultation to be sure that we are a good fit for each other. Contact me via email or through the Contact Form on this website to set up a time.

Moving forward, we’ll start with some in-depth questionnaires, interviews, and food journaling to get a picture of where you are today. Our first full-length consultation will generally be 75-90 minutes, and will include a plan of action. From there, we can decide on a schedule of re-evaluations and check-ins. If we decide that laboratory testing is indicated, we’ll order those tests after our first consultation. I also offer Pantry Raids and in-store guided shopping trips, for local clients. Just ask for more information!

My pricing is based on an hourly rate, with no commitments to packages up front. Payment is accepted via PayPal or check. I do not accept insurance.

Intake Forms

You may download the client intake forms below. (PDF Files)  Upon receipt of your completed forms, you will be invited to complete an online nutritional assessment (an in-depth analysis of your symptoms and health concerns).  You will need to complete and return all of these to me (via email or fax) no later than 48 hours prior to our first full-length appointment.
